SUBJECT:
EOP - Self Identification of Handicap
Purpose.
To remind employees of their right to identify a handicap and
distribute
Standard Form 256.
Expiration Date.
September 30, 2008
Explanation.
When
you were hired as a federal employee, you were provided a voluntary
“self identification of handicap” form to indicate whether you had any
physical impairments such as speech, hearing, vision impairments,
missing extremities, or paralysis. There are other impairments that are
listed as well, including heart disease, blood diseases, diabetes,
pulmonary or respiratory disorders, cancer, mental or emotional illness,
or learning disabilities.
Since
the day you were hired, your condition may have changed, and we are
providing SF-256 to remind you that you have a right to declare a
disability at any time during your federal employment.
The
Rehabilitation Act of 1973 requires each agency in the Executive branch
of the Federal Government to establish definite programs that will
facilitate the hiring, placement, and advancement of persons with
disabilities.
The
best means of determining agency progress in this respect is through the
production of reports showing such things as the number of persons with
disabilities that have been hired, trained, and reassigned, as well as
the number of disabled/handicapped employees at various grades and
occupations.
These
reports can help us identify deficiencies in our employment practices
that can lead us to improve conditions for our employees, as well as
help in the hiring, placement, and advancement of persons with
disabilities.
The
data collected on employees is used for the production of reports.
Please review the reverse side of the SF-256 form for other information
about the privacy of this information.
If you
want to update your self-identification form, please fill out the
attached SF256 and send it to Pat Clark, State Administrative Officer,
in a sealed envelope marked “to be opened by addressee only”.
